FOR COMPLETION
The safety of operation requires that all
questions should be answered affirmatively.
If an affirmative answer is not possible the reason should be given.
Where any questions is not considered to be applicable a note to that effect
should be inserted in the remarks column.
The presence of the letters A
and P in the column “code” indicates the following : The mentioned procedures and agreements shall be in writing and
signed by both parties;
In the case of a negative
answer the operations shall not be carried out without the permission of the
Maritime Autority.
